PURPOSE

The purpose of this investment profile questionnaire is to assist you to document your objectives and your priorities, as well as your capability to assume risk and your appetite for risk.

When completed, the information will be translated into a Valued Investor Profile which will be used to develop a Personalized Risk Management (PRISM) Portfolio.

The PRISM Portfolio is a strategic, tax-structured asset allocation model which integrates the benefits of asset diversification with the differential tax treatment associated with registered and non-registered funds and the types of investment returns generated.

ASSETS & LIABILITIES

Information regarding your total assets and liabilities is crucial to making financial projections, and therefore necessary for us to make appropriate financial recommendations.

In this section, please record the dollar amounts.  If the asset or liability belongs to your or your spouse individually, please enter the dollar amount under the appropriate person (ie, "self" or "spouse").  If the asset or liability is shared, please place the amount under the "joint" column only.  Note: the "joint" column is not a sum of the "self" and "spouse" columns, as it is possible to have amounts in all three columns.

In the "Loan Information" section, please record details regarding the loans you have indicated in the liabilities portion.

INCOME, BENEFITS & TAXATION

Central to our planning process is to determine your target income (before or after taxes).  In order to create a plan that realistically reflects your objectives, we need to know how much income you require to meet your basic annual expenses.

In the section that asks for your "Expected Family Expenses (Annual)", please indicate your average basic spending per year.  This would include things like food, utilities, clothing, annual summer vacation, etc.  The items that fall into this calculation are expenses that show up year after year.  Do not include rare, special expenses, for example, a child's university education.  Your expected family expenses can also be termed "Cash Flow Requirements".

YOUR INVESTMENT OBJECTIVES

Life Stages Explanation
* Family Commitment Phase -
Refers to the stage in life when you have a young family.  Your cash flow needs are high and saving is often limited.
* Accumulation Phase - Refers to your early career years.  Your asset base is small relative to your liabilities.  Priorities include savings, funding your children's education, purchasing a larger home, etc.  Your may be in this stage at the beginning of your income-producing years, and you may enter it again after your children have grown older.
* Consolidation Phase - Refers to the mid- to late-career stage.  Typically, your income exceeds your expenses.  For example, you may now have adult children, and therefore are no longer supporting them financially. 
* Spending and Retirement Phase - You are financially independent and no longer receive an income.  Living expenses are covered by accumulated assets such as investments and retirement benefits.
* Gifting and Estate Management Phase - You have more assets than you will need for personal spending and security for the remainder of your lifetime.  Investments at this time are seen to have an effect on not only your personal finances, but also on the estate you pass on to your beneficiaries.

* Ensuring Capital Preservation - The money you initially invested should not experience erosion.
* Reducing Management Expense Ratio - Refers to the fees involved in a managed investment.
* Improving Liquidity - Increasing your liquid assets (ie, cash) rather than your illiquid assets (ie, your home).
* Net Investment Yield - Ensuring you receive high, or consistent, returns on your investment.
* Enhancing Tax Efficiency - Reducing overall amount paid in taxes.
